Premature ejaculation is the most common sexual complaint among men, affecting an estimated one in three at some point. It's also one of the most treatable, and one of the least discussed, which is exactly why so many men live with it for years.
What it is
Clinically, PE is defined as ejaculation that consistently occurs sooner than desired (often within about a minute of penetration) accompanied by distress or interpersonal difficulty. In practice, the definition that matters is simpler: it's happening sooner than you and your partner want, and it's affecting your life.
Lifelong PE has been present since your first sexual experiences and is largely neurobiological, often involving serotonin receptor sensitivity.
Acquired PE develops after a period of normal function. This form very often has an identifiable and correctable cause: erectile dysfunction (rushing to finish before losing an erection), prostatitis, thyroid dysfunction, anxiety, or relationship stress.
Why the distinction matters
A man with acquired PE driven by early erectile dysfunction will get nowhere with technique-based approaches. Treat the ED and the PE frequently resolves on its own. This is the kind of thing a proper evaluation catches and a quick online prescription does not.
Our evaluation
- A full sexual and medical history, including onset and pattern
- Screening for erectile dysfunction, which coexists in a large share of cases
- Thyroid and hormone assessment where indicated
- Review of medications and substances
- Assessment of prostate and urinary symptoms
- An honest conversation about anxiety and relationship context
Treatment options
- Topical anesthetic agents applied before intercourse to reduce sensitivity: simple, effective, and used by many men as a first step
- Oral medication, including off-label SSRIs prescribed specifically for this purpose, which reliably increase latency
- Treating underlying ED, which resolves a large fraction of acquired cases
- Pelvic floor training, which has good evidence behind it and no side effects
- Behavioral techniques: stop-start and squeeze methods, taught properly
- Combination therapy, which generally outperforms any single approach
The part worth saying out loud
Most men wait years before raising this. Our providers treat it constantly, and the conversation takes about ten minutes. The treatments work. Schedule a confidential consultation and let's fix it.
